Thin endometrium: is it the cause of delayed pregnancy? Causes and treatments

This English text is a translation awaiting the doctor's review.
The endometrium is the inner lining of the uterus that receives the embryo after fertilisation, and its thickness and quality are decisive for a successful pregnancy. If the lining is thin or unable to support implantation, this may lead to delayed pregnancy or failed assisted reproduction attempts such as ICSI or IUI.
What is the ideal endometrial thickness for pregnancy?
The right thickness for implantation is roughly 8 to 14 mm. If the lining is thinner than 7 mm, the chances of pregnancy may fall.
Causes of a thin endometrium
- Reduced blood flow to the uterus
- Hormonal imbalance, especially low oestrogen
- Chronic inflammation of the uterine lining
- Long-term use of some contraceptives
- Repeated surgery inside the uterus, such as curettage
Symptoms that may suggest a thin endometrium
- Very light periods
- Difficulty conceiving despite regular cycles
- Repeated failure of assisted reproduction
How a thin endometrium is diagnosed
The lining is usually assessed with a transvaginal ultrasound on specific days of the cycle, measuring its thickness and pattern to judge how ready it is for pregnancy.
Available treatment options
- Hormonal treatment with oestrogen and progesterone
- Improving blood flow to the uterus with medicines or stimulation sessions
- Anti-inflammatory treatment if there is a chronic infection
- Some studies suggest a benefit from platelet-rich plasma (PRP)
When should you see a specialist?
If pregnancy has been delayed for more than a year, or you have had a failed ICSI or IUI attempt, it is advisable to see a specialist to examine the uterine lining.
